for Dental Practitioners, which includes a supply of Patient
Referral Forms together with a copy of our leaflet Patient’s
Guide to
Seeing a Specialist.
If you wish you can download a copy of the Referral
Form here.
Once you have completed the Referral Form with the
patient’s details, please forward this to us with
any relevant radiographs.
When we receive the referral we will contact the patient
directly to arrange a convenient appointment and address
any queries.
